Overview

In this unit, you will build on foundational urgent and primary care knowledge to develop extended assessment, management and referral capabilities for patients with acute and/or chronic presentations across the lifespan. The unit focuses on care of conditions across the lifespan inclusive of paediatric urgent care, chronic disease, sexual and reproductive health, mental health, addiction medicine, safeguarding and complex care coordination. You will strengthen your clinical reasoning skills, escalation of clinical decisions, person-centred care and interprofessional communication through case-based learning, tutorials and applied workshop activities.

When you finish this unit, you should be able to:

    Apply a clinical reasoning framework to enable systematic assessment of patients in primary care and/or urgent care settings.

    Apply critical thinking to recognise deterioration, red flags and escalation requirements in the management of patients  in primary care and/or urgent care settings.

    Develop evidenced based,  person-centred, culturally safe management plans for patients requiring care in urgent, primary and community-based environments.

  5. 5

    Communicate and collaborate with interprofessional teams to implement referrals, safety-netting and continuity of care across urgent, primary, community and health settings.

  6. 6

    Analyse critically clinical decision-making, professional responsibility and governance requirements in complex extended care practice.
